Allergen Matrix & Natasha’s Law Record Form

An allergen matrix template — record which of the 14 named allergens each dish contains, for customers and the EHO.

Description

Getting an allergen answer wrong can be a matter of life and death, and the law now expects you to know exactly what’s in every dish. This record makes it manageable: you mark which of the 14 named allergens appear across your menu, then log each dish against the allergens it contains and whether it can be adapted — so any team member can give a confident, correct answer at the hatch. It’s built around the 14 allergens the law requires you to declare — celery, cereals containing gluten, crustaceans, eggs, fish, lupin, milk, molluscs, mustard, peanuts, sesame, soybeans, sulphur dioxide and tree nuts. Keeping this record to hand means you meet your obligations, protect your customers and have the evidence ready when an inspector asks how you manage allergens. Use it to document your menu’s allergens and keep it updated whenever a recipe changes. Free, brandable, and the record that keeps customers safe.

Template FAQs

Common questions about the Allergen Matrix & Natasha’s Law Record Form — what it captures, when to use it, and how to make it your own.

What are the 14 allergens I must declare?

Celery, cereals containing gluten, crustaceans, eggs, fish, lupin, milk, molluscs, mustard, peanuts, sesame, soybeans, sulphur dioxide/sulphites and tree nuts. The form records which appear in each dish.

Why keep an allergen matrix?

Because the law requires you to know and declare allergens accurately, and a written record means any team member can answer correctly — and you have evidence for the EHO.

Can I note which dishes can be adapted?

Yes — there’s a column for whether a dish can be made without a given allergen, so you can offer safe alternatives with confidence.

When should I update it?

Whenever a recipe, ingredient or supplier changes. An out-of-date allergen record is worse than none, so keep it current.

Does this replace talking to the customer?

No — always confirm with the customer and never guess. The matrix makes those conversations accurate and consistent.

Is the template free?

Yes — free with a Fun Forms account and fully brandable.
