Extract System Survey & Access Audit Request Form

An extract survey template — ductwork route, access panels and grease readings scoped, so a system is assessed for TR19 compliance before any clean is quoted.

Description

Before a big or neglected extract system can be quoted properly, it needs surveying — mapping the ductwork, checking whether there’s access to clean it, and taking grease readings. This form scopes that survey, so a facilities manager or operator gets a clear picture of the system and what it’ll take to bring it to TR19 Grease compliance. It captures what a survey needs to plan around: the kitchen and system layout, how many floors the ductwork crosses, where access panels are and aren’t, suspected problem areas, and whether drawings of the system exist. The surveyor then measures grease deposits, identifies where panels must be fitted, and reports what full compliance requires — the groundwork for an accurate clean quote and a defensible fire risk assessment. Put it on your “extract survey” page for larger or inherited systems. Free, brandable, and it turns an unknown duct run into a measured, documented compliance plan.

Template FAQs

Common questions about the Extract System Survey & Access Audit Request Form — what it captures, when to use it, and how to make it your own.

What does an extract survey involve?

Mapping the ductwork route, checking access along it, measuring grease deposits with a deposit or wet-film thickness test, and identifying where access panels need fitting. You get a report on the system’s condition and what full TR19 Grease compliance will take.

Why survey before cleaning?

Because you can’t quote or clean what you can’t reach or measure. On larger or inherited systems a survey tells you the real duct route, the grease levels, and the access gaps — so the clean that follows is scoped correctly and genuinely compliant, not just the visible parts.

How is grease level measured?

With a Deposit Thickness Test or Wet Film Thickness Test, reported in microns. TR19 Grease targets a mean of 200 microns between cleans, with anything at 500 microns or more needing immediate attention. The survey records readings across the system.

What are access panels for?

They let the ductwork be inspected and cleaned internally. TR19 Grease expects panels at regular intervals — roughly every 2–3 metres where possible. The survey flags where panels are missing so they can be fitted and the whole run kept compliant.

Do you provide system drawings?

Where needed, yes — a survey can produce drawings showing the duct route and cleaned areas, which support your records and the next clean. Note on the form if you need drawings for your fire risk assessment or insurer.

Is the survey request free to send?

Yes — the form is free with a Fun Forms account, fully customizable, and it embeds on your site or shares as a link.
