Lawn Survey & Assessment Form

A lawn survey template — grass, weeds, moss and soil recorded, so a lawn’s problems are diagnosed before a treatment plan is set.

Description

A good lawn programme starts with a proper diagnosis, not a guess — and this survey records what’s actually going on. The grass type and coverage, the weeds and moss present, thatch and compaction, and the soil and shade, so the treatments that follow fix the real causes rather than just masking them. It works through what a lawn technician assesses on site: coverage and colour, the weed and moss levels, thatch depth and compaction, drainage and shade, and any pest or disease signs. A short rating across these turns a walk-over into a clear picture, and points straight to the right plan. Use it on first visits and annual reviews. Free, brandable, and it replaces “looks a bit tired” with a recorded assessment that justifies the treatments you recommend.

Template FAQs

Common questions about the Lawn Survey & Assessment Form — what it captures, when to use it, and how to make it your own.

What does a lawn survey assess?

The grass type and coverage, weeds and moss, thatch and compaction, drainage and shade, and any pest or disease signs. It diagnoses what’s wrong so the treatment plan tackles the causes, not just the symptoms.

Why survey before treating?

Because moss, weeds and thinning each have different causes — shade, compaction, poor feeding, bad drainage. Treating without diagnosing often wastes money; a survey points to what will actually work.

What is thatch and why does it matter?

Thatch is the spongy layer of dead material between the grass and soil. A little is fine; too much stops water and feed getting through and harbours disease, so measuring it decides whether scarifying is needed.

How do you tell if a lawn is compacted?

Hard, poor-draining soil where grass struggles and moss thrives is a classic sign, confirmed on site. Compaction is recorded on the survey because aeration is often the fix the lawn really needs.

Does shade affect my lawn?

A lot — heavily shaded lawns struggle to grow thick grass and favour moss, which no amount of feed fully fixes. Recording shade means realistic advice about what the lawn can achieve.

Is the survey form free?

Yes — free with a Fun Forms account, fully customisable, and ready to use on any device.
