Musician & Soloist Booking Form

A booking template for solo musicians and ensembles — capture which moments need scoring and the couple’s chosen pieces.

Description

A harpist during drinks, a guitarist for the aisle, a string quartet through the ceremony — solo and small-ensemble music is all about specific moments, and this form is built around them. It asks which part of the day needs music, what sound the couple is drawn to, and then the pieces themselves: processional, register signing, recessional. Those three song fields do a lot of quiet work. They turn a vague ‘something classical’ into named tracks you can prepare, arrange and rehearse — and they flag early if a couple has asked for something that simply doesn’t work on a solo harp. Better to know at booking than at the rehearsal. Use it whether you play solo or lead an ensemble; the sound picker covers guitar, harp, quartet, piano, sax and voice. It’s free, fully brandable, and lets couples hand you a clear, playable brief from the first click.

Template FAQs

Common questions about the Musician & Soloist Booking Form — what it captures, when to use it, and how to make it your own.

Why does the form ask about specific ceremony moments?

Because live ceremony music is moment-driven — walking in, signing the register, walking out. Capturing a piece for each means you arrive prepared for exactly the cues that matter, rather than guessing on the day.

What if a couple doesn't know what instrument they want?

The sound picker includes a ‘help me choose’ option, so undecided couples can still book and lean on your advice about what suits their venue and their taste.

Can it be used for an ensemble as well as a soloist?

Yes. The options span solo guitar and voice through to a full string quartet, so a single form works whether you perform alone or bring a group.

Why capture the exact song titles?

Because it protects everyone. Named pieces let you confirm what’s achievable, arrange them properly, and avoid a request that can’t be played convincingly on your instrument.

Does it work for parts of the day beyond the ceremony?

It does — drinks reception and the wedding breakfast are both options, so couples can book music for whichever moments they’d like scored.

Is the form free and editable?

Yes. It’s free with a Fun Forms account and every field can be edited to match the moments and instruments you offer.
