Waxing Patch Test & Skin Sensitivity Form

A waxing patch test template — record the test, any reaction and known allergies so you can wax new or sensitive skin safely.

Description

A patch test is a small step that prevents a big problem, and recording it properly matters as much as doing it. This form logs the test cleanly: when it was done, which wax was used, which area, and — a day or two later — whether the client had any reaction. It turns a quick skin check into a documented part of the client’s record. The allergy and reaction questions do the safety work. Capturing a known adhesive, fragrance or resin allergy, and whether redness or itching appeared within 24 to 48 hours, tells you plainly whether it’s safe to go ahead. A short consent line records that the client is happy to proceed if no reaction occurred, so there’s no ambiguity when they come back for the full treatment. Use it for anyone new, anyone with reactive skin, or after a long gap. It’s free, brand-styled, and gives you a tidy, dated record that you tested before you waxed.

Template FAQs

Common questions about the Waxing Patch Test & Skin Sensitivity Form — what it captures, when to use it, and how to make it your own.

When should a client have a waxing patch test?

Before a first treatment, if they have sensitive or reactive skin, or after a long break — especially with a new wax. This form records the test so you have proof you checked before treating.

What does the form capture?

The date of the test, which wax and area were used, whether a reaction appeared within 24–48 hours, and any known allergies — everything you need to decide if it’s safe to proceed.

Why record known allergies?

Because allergies to adhesives, fragrances or resin (colophony) can trigger a reaction to waxing products. Knowing them in advance lets you choose a suitable wax or avoid treatment.

What if the client reacts to the patch test?

There’s a box to describe the reaction, and the consent line only applies if no reaction occurred. A reaction means you’d hold off and discuss alternatives rather than proceed.

Does this double as consent to treat?

It records consent to proceed with waxing if the patch test was clear. For intimate treatments you’d still use a fuller consent form alongside it.

Is the template free?

Yes. It’s free with a Fun Forms account, fully brandable, and ready to embed or complete on a tablet in the salon.
